Consent to Use Video Conferencing

INFORMED CONSENT AND INFORMATION FOR VIDEOCONFERENCING OR TELEPHONE SERVICES

HOW to use DOXY.me video conferencing:

1) Confirm that this is the email address you’d like me to use to invite you to a video session.

2) Read this document in full and if you are in agreement, tick the box at the end indicating that you consent to the use of technology (teleconferencing or telephone) for therapy. Then send it back to me.

3) You will receive an email from me containing a link to join your video session at the scheduled time. After clicking the link you’ll be in a virtual “waiting room” for a minute or two until I join you.

4) You will need a computer, tablet or smartphone for the video session as well as secure access to the Internet rather than public/free Internet. Check that your Internet provider will not charge you extra for unexpected data overages, as you will be responsible for the payment of these fees.

5) It is important to be in a safe and confidential space that is free of distractions (including your cell phone).  All limits to confidentiality outlined in our Therapeutic Agreement apply to telephone and tele- video conferencing. Please review the Therapeutic Agreement.

6) In the event technical problems occur with video please call me so we can discuss continuing our session by phone.

7) If you need to cancel or change your video or phone conferencing appointment you are responsible for contacting me in advance.

8) The session will not be recorded and we will both disable any computer or device-generated recording to the best of our abilities.

9) Please talk to me if you have reservations about using this medium, if you find that video or phone conferencing is so difficult to use that it distracts from the services being provided, or if there are any other reasons why the medium seems to be causing problems in receiving services. Bringing your concerns to me will not mean that you are denied services.

10) DOXY.me is compliant with federal and provincial privacy legislation. It is an end-to-end encrypted video conferencing platform with servers located in Canada. Although the platform is a privacy-compliant software tool, in rare instances as with any communications technology there may be risks to your privacy as a result of unauthorized access.
